The compression source matters clinically, and getting it right determines the treatment. A herniated disc at L5/S1 compresses the S1 nerve root and produces calf pain and weakness when standing on tiptoe. Piriformis syndrome — where the piriformis muscle, which sits directly adjacent to the sciatic nerve in the buttock, goes into sustained spasm — mimics disc herniation exactly but doesn't require the same treatment at all.
